A Day in the Life:
As a Care Manager – Disability Services, your role will be dynamic, engaging, and rewarding. You will:
Conduct comprehensive assessments for NDIS Participants, designing and updating care plans tailored to their goals.
Coordinate with Support Workers and Allied Health Professionals, ensuring a seamless approach to participant support.
Manage incidents and risks, implementing measures to enhance safety for participants and staff.
Proactively collect feedback and resolve complaints, using insights to drive continuous improvement.
Lead and mentor a team of Disability Support Workers, promoting best practices, professional development, and a supportive work culture.

Selection Criteria:

Qualifications and Experience – Relevant qualifications in nursing, social work, or related fields, with experience in disability care management.
Care Management Skills – Ability to assess, plan, and coordinate individualised care for NDIS Participants.
Incident and Risk Management Competency – Proficiency in managing incidents and implementing proactive safety measures.
Quality Improvement Aptitude – Experience in using feedback to drive improvements and uphold service standards.
Organisational Skills – Skilled in record-keeping, time management, and multitasking.
Communication Proficiency – Advanced verbal and written skills essential for managing diverse interactions.
Leadership and Staff Development – Proven ability to mentor and coach Disability Support Workers.
Stakeholder Engagement – Demonstrated skill in handling enquiries and building collaborative relationships with participants and allied professionals.
