A Day in the Life:

As a Care Manager – Disability Services, your role will be dynamic, engaging, and rewarding. You will:

  • Conduct comprehensive assessments for NDIS Participants, designing and updating care plans tailored to their goals.

  • Coordinate with Support Workers and Allied Health Professionals, ensuring a seamless approach to participant support.

  • Manage incidents and risks, implementing measures to enhance safety for participants and staff.

  • Proactively collect feedback and resolve complaints, using insights to drive continuous improvement.

  • Lead and mentor a team of Disability Support Workers, promoting best practices, professional development, and a supportive work culture.

Selection Criteria:

  1. Qualifications and Experience – Relevant qualifications in nursing, social work, or related fields, with experience in disability care management.

  2. Care Management Skills – Ability to assess, plan, and coordinate individualised care for NDIS Participants.

  3. Incident and Risk Management Competency – Proficiency in managing incidents and implementing proactive safety measures.

  4. Quality Improvement Aptitude – Experience in using feedback to drive improvements and uphold service standards.

  5. Organisational Skills – Skilled in record-keeping, time management, and multitasking.

  6. Communication Proficiency – Advanced verbal and written skills essential for managing diverse interactions.

  7. Leadership and Staff Development – Proven ability to mentor and coach Disability Support Workers.

  8. Stakeholder Engagement – Demonstrated skill in handling enquiries and building collaborative relationships with participants and allied professionals.
