A Day in the Life:
As a Care Manager – Aged Care, your day will be diverse and fulfilling. You will:
Conduct comprehensive assessments of Aged Care Consumers, tailoring individualised care plans to their unique needs.
Coordinate daily services with a dedicated team, guiding Support Workers to provide safe, compassionate, and effective care.
Engage in regular check-ins with consumers and their families, offering support and addressing any feedback or concerns to enhance their experience.
Oversee incident management, document improvements, and implement proactive safety measures to maintain a secure environment.
Take pride in mentoring and training your team, facilitating growth and best practices to ensure excellence across the organisation.

Selection Criteria:

Qualifications and Experience – Relevant qualifications in nursing, social work, or a related field, with experience in aged care management.
Care Management Skills – Proven ability to conduct assessments, develop care plans, and support holistic care.
Incident and Risk Management Competency – Experience in managing incidents and minimising risks.
Quality Improvement Aptitude – A track record of implementing quality improvements and using feedback to enhance service delivery.
Organisational Skills – Strong skills in documentation, time management, and multitasking.
Communication Proficiency – Advanced verbal and written communication skills.
Leadership and Development – Experience in mentoring and supporting staff development.
Stakeholder Engagement – Ability to manage enquiries and foster positive relationships with consumers, allied health professionals, and other providers.
